2026 - 2027 Application for Registration

Application for Registration - The completion and submission of this form along with a $100 registration fee per child, or a $150 registration fee per family will reserve your child’s spot for the upcoming school year.
Authorization for Medical Care and Medical Release Form - Must be completed by the Parent/Guardian, signed and returned to the School Office prior to the start of the school year.
Schedule of Classes and Tuition 2026 - 2027
Universal Child Health Record - This form must be filled out by the Parent/Guardian and a Health Care Provider, and returned to the School Office prior to the start of the school year. It must be returned with a copy of the child’s most recent immunization record.
